Member Information and Beneficiary Forms

All members are responsible for keeping their member information up to date to ensure that benefits are administered correctly. This includes keeping Welfare Beneficiary (Life Insurance), Pension Beneficiary forms and Spouse, Dependent and Contact information current.

All changes to your beneficiary information require your signature and should be made at the Local 18 Benefits office. Please contact the Benefit Office for the appropriate forms.

Updates (add, edit or remove) to Spousal or Dependent information can be done at the Local 18 Benefits office or online via your Great-West Life’s Groupnet for Plan Members account.

Claims submitted for anyone that is not listed as a spouse or dependent on your benefits plan with Great West Life will not be paid.

Printable Benefit Forms

Members can submit expenses using the Healthcare Expenses Statement form or using Great-West Life’s Groupnet for Plan Members.

Please note that not all expenses can be submitted using Groupnet. For a list of what can be submitted sign in to Groupnet and click on ‘Submit a Claim’ and then click ‘Online Claim’. As of November 2013 the following expenses can be claimed online:

  • Acupuncture
  • Athletic Therapy
  • Chiropody
  • Chiropractor
  • Dental
  • Dental-Orthodontic
  • Dietician
  • Drug
  • Massage Therapy
  • Naturopath
  • Osteopath
  • Physiotherapy
  • Podiatry
  • Psychology
  • Speech Therapy
  • Vision

Great-West Life’s Groupnet for Plan Members can also be used to lookup information on your benefits and to check on the status of current eClaims.
If you have not registered for Groupnet then follow these steps to sign up:

  • Visit www.GreatWestLife.com
  • Click on Register Now
  • Fill in the form as required and click Next Step. Your six-digit plan number and four-digit Member ID will appear on any printed statements from Great-West Life.
  • Continue with the process until completed
  • Registration will be confirmed in writing via a physical letter
